Imaging department will require your name and date of birth to confirm your identity and locate your exams. Please be prepared with the date(s) and exams(s) for which you are requesting copies.

Request for transfer of room accommodation shall be facilitated at Admitting Section. The Nurse on Duty of the concerned station shall advise the patient or representative to fill-out the Request for Transfer of Room Accommodation From at the Admitting Section.
